NADH dehydrogenase subunit 4 is one of the seven mitochondrial DNA (mtDNA)-encoded subunits included among approximately 41 polypeptides of the respiratory complex I (NADH:ubiquinone oxidoreductase, EC 1.6.5.3), and is probably a component of the hydrophobic protein fragment. Complex I accepts electrons from NADH, transfers them to ubiquinone (coenzyme Q10) and uses the energy released to pump protons out across the mitochondrial inner membrane. Defects in NADH dehydrogenase subunit 4 are a cause of Leber hereditary optic neuropathy, a maternally inherited disease resulting in acute bilateral blindness due to retinal degeneration and occuring predominantly in young men. Cardiac conduction defects and neurological defects have also been described, resulting in optic nerve degeneration and cardiac dysrhythmia. More recently NADH dehydrogenase subunit 4 has also been implicated in the progression of Parkinson's Disease.
