The CTLA-4 cDNA encodes a protein known as Cytotoxic T-Lymphocyte Antigen 4 also called CD152. This protein acts as an immune checkpoint receptor. CTLA-4's molecular weight is approximately 25 kDa. It is expressed primarily on the surface of T cells particularly on activated CD4+ and CD8+ T cells and regulatory T cells. CTLA-4 contains an immunoglobulin-like extracellular domain and competes with CD28 for binding to its ligands B7-1 (CD80) and B7-2 (CD86) present on antigen-presenting cells.
Biological function summary

CTLA-4 plays a critical role in downregulating immune responses. The protein forms part of a receptor complex that transmits inhibitory signals to T cells upon ligand binding. CTLA-4 inhibits T cell proliferation and cytokine production therefore preventing overactivation of the immune system. It contributes to maintaining self-tolerance and preventing autoimmunity by modulating immune checkpoints that are essential for eluding excessive immune responses.

Pathways

Several signaling events are impacted by CTLA-4 which includes its involvement in the immune checkpoint pathway and the negative regulation of T cell receptor (TCR) signaling. The interaction of CTLA-4 with B7 molecules results in the recruitment of phosphatases such as SHP-2 that dephosphorylate signaling molecules downstream of the TCR therefore attenuating T cell activation. CTLA-4 is functionally related to proteins like PD-1 another immune checkpoint protein which further modulates pathways critical for immune equilibrium.

CTLA-4 has significant implications in autoimmune diseases like rheumatoid arthritis and type 1 diabetes. Abnormal CTLA-4 expression or function can lead to inefficient immune regulation resulting in enhanced susceptibility to these disorders. Furthermore due to its modulatory effects on immune responses CTLA-4 is an important player in tumor immune evasion mechanisms. Strategies targeting CTLA-4 such as the use of antagonist antibodies aim to enhance anti-tumor immunity similarly to treatments involving PD-1 blockade which has been promising in cancer immunotherapy.
