Intrinsic Factor also known as gastric intrinsic factor is a protein important for the absorption of vitamin B12. Produced by parietal cells in the gastric mucosa intrinsic factor has an approximate molecular weight of 45 kDa. It is secreted in the stomach and then travels to the small intestine. Intrinsic factor specifically binds to vitamin B12 forming a complex that enables the vitamin's uptake in the ileum. The presence of anti-intrinsic factor antibodies can impede its function playing a role in clinical diagnostics for conditions like pernicious anemia.
Biological function summary

The role of intrinsic factor is closely linked with vitamin B12 metabolism. As part of a transport complex it safeguards vitamin B12 from degradation as it moves through the gastrointestinal tract. In the absence of intrinsic factor vitamin B12 cannot effectively bind and undergo absorption in the ileum. The failure in this binding and absorption process often leads to severe nutritional deficiencies manifesting as a precursor to various hematological abnormalities.

Pathways

Intrinsic factor operates in the vitamin B12 absorption pathway. This pathway involves interactions with several proteins including cubilin and megalin which serve as part of the receptor-mediated endocytosis process in the ileum. Additionally the transcobalamin protein further facilitates the distribution of vitamin B12 within the bloodstream post-absorption highlighting the interconnected network between these proteins and intrinsic factor.

Intrinsic factor is strongly implicated in pernicious anemia. This condition arises when the body produces anti-intrinsic factor antibodies leading to vitamin B12 deficiency. The deficiency affects the production and maturation of red blood cells manifesting as macrocytic anemia. Moreover a lack of intrinsic factor has been associated with neurological disorders because vitamin B12 is critical for proper neural function. Without sufficient B12 myelin sheath formation and neuronal maintenance suffer which can lead to neurological complications.
