Rheumatoid arthritis 101: An overview

Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by the immune system attacking the synovial lining of joints, resulting in inflammation, hyperplastic synovium, cartilage damage, and bone erosion1.

Unlike osteoarthritis, which is caused by wear and tear, RA results from the immune system mistakenly affecting the body's tissues. This condition can lead to joint damage and deformities over time. RA affects approximately 1% of the global population, with higher prevalence in women compared to men. The exact cause of RA is still unknown, but it is believed to involve a combination of genetic, environmental, and hormonal factors2.

This article will cover the RA’s mechanisms, symptoms, diagnosis, treatment options, management strategies, and recent advances in research.

Pathological mechanisms

The pathological mechanisms of RA progress through distinct stages: triggering, maturation, targeting, and fulminant. In the triggering stage, genetic and environmental factors initiate the autoimmune response. During maturation, the immune system's attack on joint tissues intensifies, causing synovial hyperplasia. The targeting stage involves the immune system specifically attacking joint components, leading to cartilage and bone damage. Finally, the fulminant stage is marked by severe inflammation and systemic consequences3.

RA has two major subtypes based on the presence or absence of anti-citrullinated protein antibodies (ACPAs). Citrullination, catalyzed by the calcium-dependent enzyme peptidyl-arginine-deiminase (PAD), changes arginine to citrulline. ACPAs are found in 67% of RA patients, assisting in diagnosis and indicating disease progression. ACPA-positive RA is more aggressive and responds differently to treatments like methotrexate and rituximab compared to ACPA-negative RA, which has distinct genetic patterns and immune responses4.

Symptoms and diagnosis

RA typically presents symptoms such as joint pain, stiffness, swelling, and tenderness. These signs often affect multiple joints symmetrically, particularly the small joints of the hands and feet. Morning stiffness lasting more than 30 minutes is a common characteristic. Other symptoms may include fatigue, fever, and weight loss5.

Diagnosing RA involves a combination of clinical evaluation, blood tests, and imaging studies. Blood tests may reveal elevated levels of inflammatory markers like C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR), as well as the presence of rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) antibodies. Early diagnosis is crucial for initiating treatment to prevent joint damage and improve long-term outcomes6.

Treatment options

The treatment of RA aims to reduce inflammation, relieve symptoms, and prevent joint damage. Medications are a cornerstone of RA management and include nonsteroidal anti-inflammatory drugs (NSAIDs), corticosteroids, and disease-modifying antirheumatic drugs (DMARDs). DMARDs, such as methotrexate, leflunomide, and hydroxychloroquine, slow the progression of the disease and preserve joint function7.

Biologic DMARDs are advanced medications used when conventional DMARDs are ineffective. They target specific immune system components to reduce inflammation and avoid joint harm. These include tumor necrosis factor (TNF) inhibitors, which block TNF; IL-6 and IL-1 inhibitors, which target IL-6 and IL-1; and B-cell and T-cell inhibitors, which disrupt cell activation. Biologic DMARDs are typically administered via injection or infusion and are often combined with conventional DMARDs for better results.

Physical therapy and lifestyle changes, including regular exercise and a balanced diet, can help manage symptoms and improve overall health. In severe cases, surgical options such as joint replacement may be considered.

Management and living with RA

Living with RA requires a comprehensive approach to manage symptoms and maintain quality of life. Strategies for managing RA include adhering to prescribed medications, engaging in regular physical activity, and using coping mechanisms to deal with pain and fatigue. Support systems like patient support groups and counseling can provide emotional and practical assistance. RA can impact daily life and mental health, making it important to address both physical and psychological aspects of the disease. Regular follow-ups with healthcare providers are essential to monitor disease progression and adjust treatment plans as needed8.

Recent advances in RA research

Recent research in RA has led to significant advancements in understanding the disease and developing new treatments. Innovative therapies, such as Janus kinase (JAK) inhibitors and Bruton’s tyrosine kinase (BTK) inhibitors, offer new options for patients who do not respond to traditional treatments9. Clinical trials are exploring neurostimulation and omega-3 fatty acids to manage RA symptoms10. In addition, artificial intelligence (AI) and other technologies are being used to predict treatment responses and monitor patient progress. Future research aims to personalize treatment approaches further and achieve sustained remission without the need for ongoing medication.
