In this webinar, Professor Gavin Giovannoni explains how multiple sclerosis (MS) is diagnosed and how diagnostic criteria have evolved over time. Starting with the Schumacher criteria in the 1960s, which required two clinical events, he traces the progression to the Poser criteria that incorporated evoked potentials and cerebrospinal fluid analysis. The introduction of MRI revolutionized MS diagnosis, leading to the McDonald criteria, which allow earlier detection by demonstrating dissemination in time and space through imaging.
Professor Giovannoni emphasizes the importance of early diagnosis and treatment in improving outcomes and altering the natural history of MS. He also discusses prognosis, highlighting that untreated MS often leads to significant disability over decades, while modern disease-modifying therapies are creating a new “treated benign” population. Key prognostic factors include age, sex, lesion burden, brain atrophy, cognitive impairment, and biomarkers such as neurofilament levels, though their relevance may diminish with effective treatment.
